• Lead professional-practice standards, scopes of practice, policies, procedures and clinical protocols across PSW, nursing and interprofessional teams.
• Support nursing practice that enables early intervention, escalation and continuity of care in the community.
• Embed professional standards and outcome measurement into nursing-led models of care.
• Maintain compliance with regulatory-college standards, legislation, professional accountability frameworks and ethical practice.
• Lead clinical risk management, incident review, critical-incident analysis and corrective-action planning.
• Maintain relationships with regulatory bodies, professional associations and sector partners.

• Support community stabilization, ALC avoidance, chronic-disease management, dementia care and complex-care models.
Interprofessional teamwork
• Champion the One TEAM model across PSWs, nurses, care coordinators and leaders.
• Strengthen shared accountability, role clarity and psychological safety.
• Develop frontline and emerging leaders to support stronger team-based care.
Quality, safety, education and innovation
• Establish and monitor clinical quality and safety indicators such as falls, medication incidents, infections, emergency-department visits and hospital readmissions.
• Use digital tools, data and analytics to drive continuous quality improvement and proactive clinical decision-making.
• Develop a corporate clinical-education and learning strategy.
• Oversee onboarding pathways, competency frameworks and continuing professional development.
• Lead infection-prevention-and-control policies, education and risk mitigation.
• Contribute clinical expertise to funding proposals, pilots, partnerships and system initiatives.
The role is designed for a senior clinical leader who can operate at both the practice and system levels while maintaining a strong connection to frontline care.
Qualifications

• Master's degree in Nursing, Health, Education or a related field.
• 7–10 years of progressive clinical leadership experience, preferably in home/community care or integrated health systems.
• Demonstrated expertise in professional practice, quality, education and integrated models of care.
