Role overview

Trevor Frances Recruitment is recruiting an experienced Nurse Practitioner for a respected long-term-care organization in Markham. The NP functions as a clinical leader and provides advanced resident-centred care through comprehensive assessment, diagnosis, treatment planning and ongoing management while collaborating with physicians, nursing leaders and interdisciplinary teams across long-term-care services.

Clinical and leadership responsibilities

The role develops and evaluates care plans, orders and interprets diagnostic tests, prescribes/adjusts therapies within scope, coordinates referrals and specialist or virtual care, supports hospital-to-home transitions and champions palliative and end-of-life care. The NP also contributes to quality-management projects, nursing excellence, mentorship, education and best-practice development across the organization.

• Current CNO registration as a Nurse Practitioner in the Extended Class (Primary Health Care or Adult).
• Master’s Degree in Nursing.
• NP practice experience or extensive geriatric/long-term-care experience.
• Strong clinical assessment and diagnostic-reasoning skills.
• Excellent communication, conflict resolution, critical thinking and time management.
• Microsoft Office and PointClickCare or similar EHR proficiency.
• Knowledge of long-term-care legislation and standards.
• Successful Police Vulnerable Sector Check.
• LTC/geriatrics, gerontological nursing, palliative care and Chinese-language ability are listed as assets.
